Clinical Presentation & Pre-Op Summary

42yo female with Class III obesity and uncontrolled diabetes; underwent laparoscopic sleeve gastrectomy with complete glycemic remission off medications.

Operative Technique: Under general anesthesia, 4 laparoscopic ports were placed. Greater curvature mobilized from 4cm proximal to pylorus up to the angle of His. Short gastric vessels divided using ultrasonic shear. A 36-Fr calibration bougie positioned along the lesser curvature. Sequential gold and purple endo-GIA reloads deployed to create a tubular stomach. Continuous oversewing with 3-0 V-Loc barbed suture performed for hemostasis. Metabolic Resolution: Patient discharged in 48 hours. By 6 months, patient lost 32 kg of excess weight, with HbA1c falling to 5.6% without antidiabetic medications.
